1	Lost/Misplaced Term Deposit-i Receipt (Upon execution of the Indemnity Form)	For replacement of TD-i Receipt, a service charge of RM5.00 will be levied for each Receipt plus a stamp duty of RM10.00 upon execution of the 'Indemnity Form for Lost TD-i’. Note: No service charge will be levied if there is no replacement of TD-i Receipt, except for RM10.00 stamp duty for the ‘Indemnity Form for Lost TD-i’; and The above charges are not applicable for certless TD-i.
